The Pesticide Use Reporting (PUR) forms are available in PDF format:
You can download and save the PDF versions to your local system or you can fill it out on-line. In both cases, you will print it from your local printer. Remember, when using the PDF version, if you are using the free Acrobat Reader, you cannot save the data that you write into the form. If you need to make a change after printing and exiting the form, you can either reenter all of the data or hand write the updates on the printed form. (If you have purchased the full version of Adobe Acrobat, you can save PDFs along with any data entered.)
