Revised Investigative Sample Analysis Report Form DPR-ENF-030 with Updated Instructions in Compendium Volume 5, Investigation Procedures
The Investigative Sample Analysis Report and Custody Record Form, DPR-ENF-030, has been revised, along with the instructions found in Compendium Volume 5 Investigation Procedures, Chapter III.A.10 Sample Collection.
Summary of Revisions:
- Some sections were moved, renamed, or rearranged for clarity and to reduce redundancy.
- Section C. Expanded the information on sample type (e.g., for dislodgeable samples, added the number and size of leaf punches.)
- Section D1. Added a line to request alternate discard date, beyond the standard 3 months.
- Section D2. Added a line for the laboratory to indicate acceptability of sample.
- Section E. The new pesticide screens using GC/MS (Gas Chromatograph/Mass Spectrometer) and LC/MS (Liquid Chromatograph/Mass Spectrometer) are added, at the request of the California Department of Food & Agriculture Center for Analytical Chemistry (CDFA Laboratory).
- Section J. Added lines to document the full chain-of-custody and for storage location. Added a signature block for the Sacramento CDFA Lab receipt of samples.
- Added a glossary of terms and abbreviations.
Please begin using the enclosed revised Investigative Sample form DPR-ENF-030 (Rev. 03/16) found on the DPR forms page at www.cdpr.ca.gov/docs/enforce/prenffrm/dpr-enf-030.pdf. Also enclosed are the revised instructions for this form found in Compendium Volume 5, Investigation Procedures, Chapter III-A.10, Sample Collection, pages 53-57.
