Notification to Order Revised Pesticide Use Report Forms for Field Fumigant Applications

To assist in the collection of pesticide use data associated with applications of field fumigations in nonattainment areas, the Department of Pesticide Regulation (DPR) has revised the following pesticide use report (PUR) forms to capture the field fumigation method code associated with these applications:

1. Production Agriculture Monthly Pesticide Use Report, PR-ENF-017A (processor use)
2. Production Agriculture Monthly Pesticide Use Report Supplement, PR-ENF-017B (processor use)
3. Production Agriculture Monthly Pesticide Use Report, PR-ENF-017C
4. Pesticide Use Report, PR-ENF-025
5. Notice of Intent to Apply Restricted Materials, PR-ENF-126X
6. Production Agriculture Monthly Pesticide Use Report: Multiple Site/Commodity, PR-ENF-183
7. Production Agriculture Monthly Pesticide Use Report: Multiple Section, PR-ENF-184

The revised PUR forms should be distributed to growers and pest control operators for their use beginning in January 2009.

As you are aware, DPR delayed printing and distribution of the revised PUR forms pending completion of modifications to all data entry and associated permit/PUR databases. The forms are ready to send to the Office of State Publishing and should be available for distribution to the counties by mid-November, 2008.

Enclosed, you will find an abbreviated order form listing these revised PUR forms. So that we can distribute an accurate number of forms to each county, would you please complete the order form and fax it to Enforcement Branch, Attention Linda Lichtenberger at 916-445-3907 by Friday, October 16, 2008.
