INSPECTION FORMS ANNOUNCEMENTS

The enclosed document explains particulars about the delivery of the revised inspection forms to counties by mid-June.

The use of these revised inspection forms by all counties becomes effective July 1, 2003. With the exception of the Pesticide Pre-Application Site Inspections (PR-ENF-102, Est. 3/03), all new forms are to be submitted along with the Pesticide Regulatory Activities Monthly Report (Report 5) beginning with the July 2003, Report 5. Older forms are to be discarded, and will not be accepted after July 1, 2003.

Inspection forms submitted along with each monthly Report 5 will be data entered for the Data Evaluation program through the end of fiscal year 2003/04. A determination to continue the program will be made prior to the end of that fiscal year.
