Revised Forms Requisition
Enclosed are the revised forms requisition (PR-ENF-197) and a listing of all new, discontinued, and revised forms since the last forms requisition (Rev. 01/05) was distributed in Enforcement Letter 05-01. Some changes/reminders to note:
During the Spring 2005 CACASA Conference, an updated version of the agreement-as drafted by DPR in consultation with CACASA and U.S. EPA Region 9-was approved and a representative of each party has signed it.
- Form orders will be sent to the Forms Coordinator either by fax (916) 445-6845, mail (see letterhead address), or e-mail at formscoordinator@cdpr.ca.gov.
- All questions regarding requisitions should be addressed to the Forms Coordinator at (916) 323-8367, or e-mail at formscoordinator@cdpr.ca.gov.
- If faxing requisition, please provide number of pages included in fax.
- When sending in new order, please do not include previously ordered amounts in new order.
ABSOLUTELY NO FORM ORDERS WILL BE TAKEN OVER THE PHONE in order to accurately track orders. PLEASE FAX, MAIL, OR E-MAIL ALL FORM ORDERS.
