Revised Inspection Forms – Quantity Estimates Needed from Counties for Fiscal Year 2003/04

The Pesticide Regulatory Affairs Committee approved revisions to the various inspection forms used in the pesticide regulatory program at the 2002 California Agricultural Commissioners and Sealers Association Winter Conference.

Use of these forms becomes effective July 1, 2003, and will supercede those currently being used by counties statewide. A conversion table indicating current (“old”) forms and revised forms is included (see enclosure 1). Also enclosed is a copy of each revised or new inspection form; twelve forms in all. All inspections after July 1, 2003, must be submitted on the newly revised forms to accommodate the Data Evaluation Report.

Please submit your best estimate of the quantity of each type of form needed to meet your needs for the entire fiscal year 2003/04 to the Department of Pesticide Regulation by February 14, 2003 (see enclosure 2). The forms will be printed with sequential serial numbers, on 4 part no-carbon-required paper. Serial numbers are being added to assist with tracking.
