Revised Investigative Sample Analysis Report Form DPR-ENF-030

The Investigative Sample Analysis Report and Custody Record Form, DPR-ENF-030, has been revised. Please begin using the enclosed DPR-ENF-030 (Rev. 03/21). This form is also available on the DPR forms page at www.cdpr.ca.gov/docs/enforce/prenffrm/dpr-enf-030.pdf.

The revisions to the form are:

• Section I – Updated the contact information for the Northern Regional Office
• Section J – Broadened the laboratory common carrier certification statement to apply to any laboratory
• Abbreviation of Terms – Removed outdated detection code 01 and extraction code 999

These revisions do not affect the instructions found in Compendium Volume 5 Investigation Procedures, Chapter III.A.10 Sample Collection. If you have any further questions, please contact the Enforcement Branch Liaison assigned to your county.
